Liability form used for youth group ski trips to Cascade Mtn. Each minor will also need a completed church permission slip form located on this page too.
Chaperone waiver form to be kept on file for any adult chaperoning overnight youth activities. Form is used to help meet our safe sacturary policy. A copy should be given to the pastor for long-term records.
Driver waiver form to be kept on file for anyone that chauffers groups of people for church activities (youth group, etc.) Give a copy to the pastor for long term record.
